1. That I am the deponent herein and I am well aware of the facts of this affidavit.

1. That I am deponent herein and I am well aware of the facts of this affidavit.
2. That I am holder of an Indian Passport bearing number G6670251, wherein my name is mentioned as SUBAIDA MUTHUKOYA instead of SUBAIDA K C. 3. That I also hold Aadhar Card with Unique Identification No.6644 0464 3622 issued by the Govt of India, in which my name is recorded correctly as SUBAIDA K C. 4. That due to inadvertence and clerical error, there is a discrepancy in my name as recorded in the Passport and Aadhar Card.
5. That both the names SUBAIDA MUTHUKOYA instead of SUBAIDA K C refer to one and the same person, i.e., myself.
6. That I am making this affidavit to declare and confirm that my correct name which I wish to be officially recognized, is SUBAIDA K C only.
7. That I undertake to use my correct name as SUBAIDA K C in all my future records and documents.
8 That I am executing this affidavit to be submitted to the concerned authorities for the purpose of rectification of my name in passport and various records and for publication in the Gazette, as required.
