AFFIDAVIT
I, SAIDA DATHIFANUGOTHI, D/o Hussain Kudathigothi, aged 54 years, residing at Dathifanugothi, Kudehi Village, Minicoy Island, U.T. of Lakshadweep-682559 do hereby solemnly affirm and state as follows:-
1. That I am the deponent herein and I am well aware of the facts of this affidavit.
2. That I am a holder of Aadhar Card bearing number 9500 8109 3001, issued by Government of India wherein my name is mentioned as SAIDHA DATHIFANUNGOTHI instead of SAIDA DATHIFANUGOTHI.
3. That I also hold PAN Card EZHPS3568B issued by Government of India wherein my name is recorded as SAIDA only, instead of SAIDA DATHIFANUGOTHI.
4. That I also hold an Indian Passport bearing No.X5677760 issued by Government of India in which my name is recorded correctly as SAIDA DATHIFANUGOTHI.
5. That I also hold Birth Certificate issued by Registrar of Birth and Death, Minicoy Island, in which my name is recorded correctly as SAIDA DATHIFANUGOTHI.
6. That due to inadvertence and clerical error, there is a discrepancy in my name as recorded in the Aadhar Card and PAN Card.
7. That both names SAIDHA DATHIFANUNGOTHI, SAIDA instead of SAIDA DATHIFANUGOTHI are refer to one and same person, ie; myself.
8. That I am executing this affidavit to declare and confirm that my correct name which I wish to be officially recognised is SAIDA DATHIFANUGOTHI only.
9. That I undertake to use my correct name as SAIDA DATHIFANUGOTHI in all my future records and documents.
THE LAKSHADWEEP GAZETTE 210. That I am executing this affidavit to be submitted to the concerned authorities for the purpose of rectification of my name in Aadhar Card and PAN Card and for publication in the Official Gazette, as required.
