एफ 10-02 / 2021 /सत्रह /मेडि-2 यतः सेवाओं अथवा प्रसुविधाओं अथवा सहायकिओं के परिदान के लिए पहचान दस्तावेज रूप में आधार का उपयोग सरकारी परिदान की प्रक्रियाओं को सरल करता है , इसमें पारदर्शिता तथा दक्षता लाता है और लाभार्थियों को उनके हकों को सुविधाजनक तथा निर्बाध रीति से सीधे प्राप्त करने में समर्थ बनाता है तथा आधार किसी व्यक्ति की पहचान सिद्ध करने हेतु विभिन्न दस्तावेज प्रस्तुत करने की आवश्यकता से छुटकारा दिलाता है ।
और यतः, लोक स्वास्थ्य एवं परिवार कल्याण विभाग , मध्यप्रदेश (जिसे इसमें इसके पश्चात विभाग के नाम से निर्दिष्ट किया गया है) द्वारा ई .के.वाई.सी. हेतु आधार प्रमाणीकरण सेवाओं का उपयोग आहरण और संवितरण अधिकारी (डी.डी.ओ.) / ई-वित में भुगतान अनुमोदनकर्ताओं के जनसंख्यिकीय ब्यौरे सुनिश्चित करने , ई- वित में भुगतान अनुमोदनकर्ताओं एवं अल्ट्रासोनोग्रॉफी करने वाले चिकित्सकों के बायोमेट्रिक प्रमाणीकरण एवं पी.सी.पी. एन.डी.टी एम.आई.एस. में प्रविष्टि करने के लिए किया जा रहा है ,
और यतः, कार्यान्वयन अभिकरणों (एजेंसियों) के माध्यम से दी जाने वाली पूर्वोक्त सेवाएं भारत की संचित निधि से उपगत व्यय अंतर्ग्रस्त करती है ।
अतएव , आधार (वित्तीय और अन्य सहायिकियों, प्रसुविधाओं और सेवाओं का लक्षित परिदान ) अधिनियम , 2016 (2016 का 18) (जिसे इसके पश्चात उक्त अधिनियम के नाम से निर्दिष्ट किया गया है) , की धारा 7 के उपबंधों के अनुसरण में, राज्य शासन , एतदद्वारा , निम्नलिखित को अधिसूचित करता है, अर्थातः-
1. ( 1 ) सेवा प्राप्त करने के इच्छुक किसी पात्र व्यक्ति से आधार का प्रमाण प्रस्तुत किया जाना अथवा आधार प्रमाणीकरण से होकर गुजरना अपेक्षित है ।

(2) सेवा प्राप्त करने का इच्छुक किसी व्यक्ति से , जिनके पास आधार नहीं है या जिसने अभी तक आधार हेतु नामांकन नहीं कराया है , से एतदद्वारा इस अधिसूचना के 02 माह के भीतर आधार नामांकन हेतु आवेदन किया जाना अपेक्षित है , बशर्ते कि वह आधार अधिनियम की धारा 3 के उपबंधों के अनुसार आधार प्राप्त करने का हकदार हो और ऐसा व्यक्ति आधार के लिए नामांकन कराने हेतु किसी भी आधार नामांकन केन्द्र (सूची भारतीय विशिष्ट पहचान प्राधिकरण (यू.आई.डी.ए.आई) वेबसाइट www.uidai.gov.in पर उपलब्ध है) में जा सकेगा ।
(3) आधार (नामांकन तथा अद्यतन) विनियम , 2016 के विनियम 12 के अनुसार , संबंधित मंत्रालय द्वारा अपने निगमों के माध्यम से , जो किसी व्यक्ति से आधार प्रस्तुत करने की अपेक्षा करते है । उन लाभार्थियों के लिए आधार नामांकन सुविधाएं प्रदान किया जाना अपेक्षित है , जो आधार के लिए नामांकित नही है और यदि संबंधित ब्लॉक या तालुका या तहसील में कोई आधार नामांकन केन्द्र नहीं है , वहां विभाग से, भारतीय विशिष्ट पहचान प्राधिकरण (यू.आई.डी.ए.आई) के विद्यमान पंजीयकों के साथ समन्वय से या स्वयं (यू.आई.डी.ए.आई) रजिस्ट्रार बनकर सुविधाजनक स्थानों पर आधार नामांकन सुविधाएं प्रदान किया जाना अपेक्षित है ।
परन्तु व्यक्ति को आधार सौंपे जाने के समय तक ऐसे व्यक्ति द्वारा सेवायें हेतु निम्नलिखित पहचान दस्तावेज प्रस्तुत करने के अध्यधीन रहते हुए प्रदान की जाएंगी , अर्थातः-
(क) (एक) यदि उसने अपने आपको नामांकित किया है तो उसकी आधार नामांकन आई.डी पर्ची , अथवा
(दो) पैरा 2 के उप-पैरा (ख) में यथाविनिर्दिष्ट आधर नामांकन के लिए किए गए उसके अनुरोध की एक प्रति; और ।
(ख) (एक) फोटो सहित बैंक पासबुक; या
(दो) भारत निर्वाचन आयोग द्वारा जारी मतदाता पहचान पत्र; या
(तीन) राशनकार्ड;या
(चार) आयकर विभाग द्वारा जारी स्थायी खाता संख्या (पैन) कार्ड ;या
(पांच) पासपोर्ट; या
(छह) किसी राजपत्रित अधिकारी द्वारा शासकीय लेटर हैड पर जारी ऐसे सदस्य की फोटो वाला पहचान का प्रमाण- पत्र; या
(सात) मंत्रालय द्वारा विनिर्दिष्ट कोई अन्य दस्तावेजः
परंतु यह और कि उपरोक्त दस्तावेजों की जांच इस प्रयोजन हेतु विभाग द्वारा नामनिर्दिष्ट अधिकारी द्वारा की जाएगी ।

2. सुविधाजनक और कठिनाई रहित सेवा प्रदान करने के लिए , विभाग आवश्यक व्यवस्थाएं सम्मिलित करेगा , जिसमें निम्नलिखित सम्मिलित है , अर्थातः-
(क) मीडिया के माध्यम से व्यापक प्रचार तथा मीडिया के माध्यम से व्यापक प्रचार तथा सेवा प्रदाता को योजना के अधीन आधार की आवश्यकता के बारे में जागरूक करने के लिए मीडिया के माध्यम से व्यक्तिगत सूचना प्रदान की जाएगी तथा उनके पहले से ही नामांकित न होने की दशा में इस अधिसूचना के 2 माह भीतर उनके उनके क्षेत्रो में उपलब्ध निकटतम आधार नामांकन केन्द्र मे अपना नामांकन कराने के लिए उन्हे सलाह दी जा सकेगी । स्थानीय स्तर पर उपलब्ध नामांकन केन्द्रों की सूची (www.uidai.gov.in पर उपलब्ध सूची) उन्हे उपलब्ध कराई जाएगी ।
(ख) यदि आसपास, जैसे ब्लॉक या तालुका या तहसील में नामांकन केन्द्रों की अनुपलब्धता के कारण सेवा प्रदाता अपना आधार नामांकित नहीं करा पाते हैं , तो विभाग से सुविधाजनक स्थानों पर आधार नामांकन सुविधाओं को सुकर बनाना अपेक्षित है , तथा सेवा प्रदाताओं से , पैरा 1 के उप -पैरा (3) के प्रथम परंतुक में यथाविनिर्दिष्ट ब्यौरों के साथ अपने नाम , पते , मोबाईल नंबर विभाग के संबंधित अधिकारी को या इस प्रयोजन के लिए उपलब्ध वेब पोर्टल के माध्यम से प्रदान कर आधार नामांकन के लिए उनका अनुरोध रजिस्टर कराने का अनुरोध किया जा सकाता हैं ।.

F 10-02/ 2022 / 17/ Medi - 2 : -Whereas , the use of Aadhaar as identity document for delivery of services or benefits or subsidies simplifies the Government delivery processes , brings in transparency and efficiency, and enables beneficiaries to get their entitlements directly in a convenient and seamless manner and Aadhaar obviates the need for producing multiple documents to prove one's identity;
And whereas , the Department of Public Health and Family Welfare ( hereinafter referred to as the Department) , Madhya Pradesh is using Aadhaar Authentication services for eKYC for ascertaining demographic details of Drawing and Disbursing Officer ( DDOs ) / payment approvers in e - Vitt, biometric authentication of payment approvers in e - vitt and medical practitioners conducting ultrasonography and making entries in PCPNDT MIS .
And whereas , the aforesaid services are offered through the Implementing Agencies involves expenditure incurred from the Consolidated Fund of India;
Now, therefore, in pursuance of the provisions of Section 7 of the Aadhaar (Targeted Delivery of Financial and other Subsidies , Benefits and Services ) Act, 2016 ( 18 of 2016) ( herein after referred to as the said Act) , the State Government hereby notifies the following , namely : -
1. ( 1 ) An eligible Individual desirous of availing services is required to furnish proof of possession of Aadhaar or undergo Aadhaar authentication ;
(2) An individual desirous of availing services , who does not possess an Aadhaar or has not yet enrolled for Aadhaar is hereby required to apply for Aadhaar enrolment within 02 months of notification, provided he or she is entitled to obtain Aadhaar as per the Section 3 of the Act and such individual may visit any Aadhaar enrolment centre ( list available at Unique Identification Authority of India ( UIDIA ) website www.uidai.gov.in for Aadhaar enrolment;

(3) As per regulation 12 of the Aadhaar (Enrolment and Update) Regulation , 2016 , the concerned Ministry through its Corporations which requires an individual to furnish Aadhaar , is required to offer Aadhaar enrolment facilities for the beneficiaries who are not enrolled for Aadhaar and in case there is no Aadhaar enrolment centre located in the respective Block or Taluka or Tehsil , the Department is required to provide Aadhaar enrolment facilities at convenient locations in coordination with the existing Registrars of Unique Identification Authority of India ( UIDAI ) or by becoming UIDIA Registrars themselves ;
Provides that till the time Aadhaar is assigned to the individual , services shall be given by such individuals subject to the production of the following identification documents , namely: -
(a) (i ) Aadhaar Enrolment ID slip if she or he has enrolled or herself; or
(ii) a copy of her of his request made for the Aadhar enrolment , as specified in sub paragraph (b ) or paragraph 2;
: and
(b) (i ) Bank passbook with photography ; or (ii ) Voter identity card issued by the Election Commission of India or (iii ) Ration Card , or ( iv ) Permanent Account Number (PAN) Card issued by the Income Tax Department; or ( v) Passport ; or (vi ) Certificate of identity having photo of such member issued by a Gazetted Officers on an official letter head ; or ( vii ) any other documents specified by the Ministry ;
Provided further that the above documents shall be checked by an officer designated by the Department for that purpose .
2. In order to provide convenient and hassle free service , the Department shall make the required arrangements including the following , namely: -
a) Wide publicity through media and individual notices through media and individual notices through the Department , shall be given to the services provider to make them aware of the requirement of Aadhaar under the Schemes and they may be advised to ge themselves enrolled at the nearest Aadhaar enrolment centers available in their areas: within 2 months of notification , in case they are not already enrolled . The list of locally available enrolment centers (list available at www.uidai.gov.in) shall be made available to them .
b) In case, the service providers are not able to enrol for Aadhaar due to non - availability of enrolment centers within the vicinity such as in Block or Taluka or Tehsil , the Department is required to facilitate Aadhaar enrolment facilities at convenient locations , and the service providers can be requested to register their request for Aadhaar enrolment by giving their names , addresses , mobile numbers with those details as specified in the first proviso to sub - paragraph ( 3) of paragraph 1 , to the concerned official of the Department or through the web portal provided for the purpose .
3. This notification shall come into effect from the date of its publications in the Officia ! Gazette in all the States .
