यतः , पहचान स्थाचित करने के लिए आधार नंबर का उपयोग व्यक्तियों को सुविधाजनक और निर्बाध तरीके से सब्सिडी, लाभ और सेवाएं प्राप्त करने में सक्षम बनाता है, पहचान स्थापित करने के लिए दस्तावेजों की बहुतायत की आवश्यकता से छुटकारा दिलाता है, प्रक्रियाओं को सरल बनाता है और पारदर्शिता और दक्षता को बढ़ावा देता है:

और यतः , मध्यप्रदेश शासन के अधीन विभिन्न विभाग ( जिन्हें इसमें इसके पश्चात् यथास्थिति उक्त मंत्रालय या उक्त विभाग या उक्त सरकार के रूप में निर्दिष्ट किया गया है ) लाभार्थी पंजीकरण और सत्यापन के प्रयोजनों के लिए समग्र प्लेटफॉर्म पर समग्र आईडी का उपयोग करके विभिन्न विभागीय योजनाओं/सेवाओं को संचालित कर रहे हैं ( योजनाएं परिशिष्ट में संलग्न किए गए अनुसार);

और यतः , सब्सिडी, लाभ या सेवा जिनकी प्राप्ति के लिए आधार नंबर का प्रमाण आवश्यक किया जाना प्रस्तावित है . व्यक्ति/लाभार्थी को उक्त योजना और उसके संबंध में जारी निदेशों और दिशा-निर्देशों के अधीन दिया जाता है । उक्त योजना के लिए व्यय मध्यप्रदेश राज्य की संचित निधि से उपगत किया जाता है;

अतएव, सुशासन के लिए आधार अधिप्रमाणन (समाज कल्याण, नवाचार, ज्ञान ) नियमावली, 2020 के नियम 5 के साथ पठित आधार (वित्तीय और अन्य सहायिकियों, लाभों और सेवाओं का लक्ष्यित परिदान ) अधिनियम. 2016 (2016 का 18 ) की धारा 7 द्वारा प्रदत्त शक्तियों को प्रयोग में लाते हुए , राज्य सरकार , एतद्द्‌वारा, उक्त लाभों की प्राप्ति हेतु शर्त के रूप में किसी लाभार्थी की पहचान स्थापित करने के प्रयोजन के लिए आधार नम्बर के प्रयोग को अधिसूचित करती है और

अपेक्षा करती है कि ऐसा लाभार्थी अधिप्रमाणन करेगा या आधार नम्बर के होने का प्रमाण प्रस्तुत करेगा या किसी ऐसे व्यक्ति की दशा में जिसके पास उसे समनुदिष्ट आधार नम्बर नहीं है, वह इस निमित्त जारी दिशा निर्देशों के अनुसार नामांकन हेतु आवेदन प्रस्तुत करेगा, अर्थात्ः-

1. ( 1 ) उक्त योजना के अधीन उक्त लाभ प्राप्त करने के इच्छुक किसी व्यक्ति से अधिप्रमाणन कराने या आधार नंबर रखने का प्रमाण प्रस्तुत करने की अपेक्षा की जाएगी ।

(2) किसी व्यक्ति को आधार नंबर समनुदिष्ट नहीं किए जाने की स्थिति में उससे नामांकन के लिए आवेदन करने की अपेक्षा की जाएगीः

परंतु यदि वह व्यक्ति बच्चा है , तो ऐसा आवेदन केवल उसके माता- पिता या विधिक अभिभावक की सहमति से प्रस्तुत किया जाएगा।

(3) आधार (नामांकन और अद्यतन) विनियम , 2016 के विनियम 12 के उपबंधों के अनुसार , मध्यप्रदेश शासन के अधीन उक्त विभाग ऐसे लाभार्थियों का नामांकन सुनिश्चित करेंगे , जिनका अभी नामांकन किया जाना है अथवा रजिस्ट्रार के साथ समन्वय कर और सुविधाजनक स्थानों पर नामांकन केन्द्रों की स्थापना या स्वयं रजिस्ट्रार बनकर नामांकन सुविधाएं प्रदान करने सहित समुचित उपायों के माध्यम से उनके आधार विवरण को अद्यतन करेंगे:

परंतु ऐसे लाभार्थी को आधार नंबर समनुदिष्ट किए जाने तक वह निम्नलिखित दस्तावेज प्रस्तुत करके , जिनका कि वह हकदार है और जो प्रस्तुत किए जाने के समय विधिमान्य हैं, उक्त लाभ प्राप्त करने के लिए अपनी पहचान स्थापित कर सकेगा या यदि मध्यप्रदेश शासन के अधीन उक्त विभागों द्वारा ऐसी पहचान के लिए प्रदान किया गया या अधिकृत किया गया सॉफ्टवेयर ऐसे दस्तावेजों की अन्तर्वस्तुओं को प्रमाणित करने के प्रयोजन के लिए उनकी तैयारी या रखरखाव से संबंधित प्राधिकरण के डेटाबेस से इलेक्ट्रॉनिक जानकारी प्राप्त करने का, ऐसी प्राप्ति करने के लिए अपनी सहमति देकर , समर्थन करता है , अर्थात्: -

18 वर्ष से कम आयु के ऐसे बच्चे के लिए जिसे आधार नंबर समनुदिष्ट नहीं किया गया है:

(क) लाभार्थी के नामांकन केंद्र पर ऑपरेटर द्वारा प्रदान की गई नामांकन की प्रक्रिया से गुजरने की पावती, जिसमें नामांकन आईडी (ईआईडी) अंतर्विष्ट है;

(ख) निम्नलिखित दस्तावेजों में से कोई एक, यह प्रमाणित करने के लिए कि लाभार्थी 18 वर्ष से कम आयु का बच्चा है, अर्थात्ः-

(एक) जन्म और मृत्यु पंजीकरण अधिनियम, 1969 ( 1969 का 18 ) के अधीन प्रदान किया गया जन्म प्रमाण- पत्र, लाभार्थी के जन्म के संबंध में जन्म रजिस्टर में की गई प्रविष्टि से उद्धृत;

(दो) भारतीय पासपोर्ट;

(तीन) किसी मान्यताप्राप्त स्कूल शिक्षा बोर्ड द्वारा जारी मैट्रिकुलेशन या 10वीं कक्षा या उच्चतर माध्यमिक या 12वीं कक्षा का प्रमाण- पत्र या अंकसूची;

(चार ) देखरेख और संरक्षण की आवश्यकता वाले बच्चे या कानूनी मामले में संलिप्त बच्चे के संबंध में, जिसे किशोर न्याय ( बालकों की देखरेख और संरक्षण ) अधिनियम, 2015 (2016 का 2 ) के अधीन उसके द्वारा बनाए गए नियमों के अधीन संबंधित राज्य शासन के साथ पंजीकृत चाइल्ड केयर इंस्टीट्यूट में रखा गया हो , ऐसी संस्था के प्रभारी व्यक्ति, अधीक्षक, बाल कल्याण अधिकारी या परिवीक्षा अधिकारी द्वारा जारी उसकी जन्मतिथि निर्दिष्ट करने वाला प्रमाण- पत्र; या

(पांच) विदेशी नागरिक के संबंध में, -

(क) यदि वह ओवरसीज सिटीजन ऑफ इंडिया कार्डधारक है , तो ओवरसीज सिटीजन ऑफ इंडिया कार्ड ;

(ख) यदि वह तिब्बती शरणार्थी है , तो विदेशी क्षेत्रीय पंजीकरण कार्यालय (फारेनर्स रीजनल रजिस्ट्रेशन आफिस ) द्वारा जारी पंजीकरण प्रमाण- पत्र;

( ग) यदि वह नेपाल या भूटान का नागरिक है, तो नेपाल या भूटान का पासपोर्ट ;

(घ) यदि वह ओवरसीज सिटीजन ऑफ इंडिया कार्डधारक, तिब्बती शरणार्थी या नेपाल या भूटान के नागरिक से भिन्न है , तो विदेशी पासपोर्ट के साथ भारतीय वीजा या वर्तमान में विधिमान्य या विधिमान्यता समाप्त हो चुके विदेशी पासपोर्ट के साथ भारत का दीर्घकालिक वीजा; या

( ग) लाभार्थी की फोटोयुक्त निम्नलिखित दस्तावेजों में से कोई एक, जो उसके माता- पिता या विधिक अभिभावक के साथ उसके संबंध को प्रमाणित करता हो , अर्थात्ः -

(एक) राशन कार्ड ;

(दो) राजपत्रित अधिकारी जो कार्यपालिक मजिस्ट्रेट या राज्य शासन के तहसीलदार से अनिम्न श्रेणी के राजस्व अधिकारी द्वारा जारी जाति प्रमाण - पत्र या निवास प्रमाण-पत्र;

(तीन) किसी शासकीय संस्था या सार्वजनिक क्षेत्र के उद्यम द्वारा किसी सेवानिवृत्त या सेवारत लोक सेवक या उसके परिवार के सदस्य को जारी किया गया चिकित्सा या बीमा पहचान पत्र;

(चार) भारतीय पासपोर्ट;

( पांच ) किसी मान्यता प्राप्त स्कूल शिक्षा बोर्ड द्वारा जारी मैट्रिकुलेशन या 10वीं कक्षा या उच्चतर माध्यमिक या 12वीं कक्षा का प्रमाण -पत्र या अंकसूची;

(छह) देखरेख और संरक्षण की आवश्यकता वाले बच्चे या विधिक मामले में संलिप्त बच्चे के संबंध में, जिसे किशोर न्याय ( बालकों की देखरेख और संरक्षण) अधिनियम , 2015 (2016 का 2) के अधीन उसके द्वारा बनाए गए नियमों के अधीन संबंधित राज्य शासन के साथ पंजीकृत चाइल्ड केयर इंस्टीट्यूट में रखा गया हो , ऐसी संस्था के प्रभारी व्यक्ति, अधीक्षक, बाल कल्याण अधिकारी या परिवीक्षा अधिकारी द्वारा जारी उसकी जन्म तिथि निर्दिष्ट करने वाला प्रमाण- पत्र; या मध्यप्रदेश राजपत्र, दिनांक 15 अप्रैल 2025

(सात) विदेशी नागरिक के संबंध में , -

(क) यदि वह ओवरसीज सिटीजन ऑफ इंडिया कार्डधारक है , तो ओवरसीज सिटीजन ऑफ इंडिया का कार्ड ;

(ख) यदि वह तिब्बती शरणार्थी है, तो विदेशी क्षेत्रीय पंजीकरण कार्यालय फारेनर रीजनल रजिस्ट्रेशन आफिस द्वारा जारी पंजीकरण प्रमाण-पत्र;

( ग) यदि वह नेपाल या भूटान का नागरिक है , तो नेपाल या भूटान का पासपोर्ट;

(घ) यदि वह ओवरसीज सिटीजन ऑफ इंडिया कार्डधारक, तिब्बती शरणार्थी या नेपाल या भूटान का नागरिक से भिन्न है, तो विदेशी पासपोर्ट के साथ भारतीय वीजा या वर्तमान में वैध या वैधता समाप्त हो चुके विदेशी पासपोर्ट के साथ भारत का दीर्घकालिक वीजा; या

(आठ) ऐसे लाभार्थी के संबंध में , जिसके पास विधिक संरक्षक है , विधिक संरक्षकता के साक्ष्य हेतु दत्तक ग्रहण आदेश या अन्य दस्तावेज , जो संरक्षक एवं प्रतिपाल्य अधिनियम , 1890 ( 1890 क 8 ) किशोर न्याय ( बालकों की देखरेख और संरक्षण ) अधिनियम , 2015 (2016 का 2) राष्ट्रीय स्वपरायणता प्रमस्तिष्कघात, मानसिक मंदता और बहुनिशक्तताग्रस्त व्यक्ति कल्याण न्यास अधिनियम , 1999 ( 1999 का 44) या दिव्यांगजन अधिकार अधिनियम, 2016 (2016 का 49 ) और उक्त अधिनियमों के अधीन बनाए गए लागू नियमों एवं विनियमों के अधीन विधि के न्यायालय या सक्षम प्राधिकारी द्वारा जारी किया गया हो; या

(नौ) कोई अन्य दस्तावेज, जो कि राज्य शासन के अधीन उक्त विभाग निर्दिष्ट करे ।

18 वर्ष या उससे अधिक आयु के लाभार्थियों के लिए, जिन्हें आधार नंबर समनुदिष्ट नहीं किया गया है:

(क) लाभार्थी के नामांकन केंद्र पर ऑपरेटर द्वारा प्रदान की गई नामांकन की प्रक्रिया से गुजरने की पावती, जिसमें ईआईडी अंतर्विष्ट हो;

(ख) लाभार्थी की तस्वीर वाले निम्नलिखित दस्तावेजों में से कोई एक दस्तावेज अर्थात्ः-

(एक) भारत के चुनाव आयोग द्वारा जारी मतदाता फोटो पहचान पत्र;

(दो) राशन कार्ड;

(तीन) राजपत्रित अधिकारी , जो कार्यपालिक मजिस्ट्रेट या तहसीलदार से अनिम्न पद श्रेणी का राज्य शासन का राजस्व अधिकारी हो , द्वारा जारी जाति प्रमाण - पत्र या निवास प्रमाण-पत्र;

(चार ) किसी शासकीय सत्ता या सार्वजनिक क्षेत्र के उद्यम द्वारा सेवानिवृत्त या सेवारत् लोक सेवक या उसके परिवार के सदस्य को जारी किया गया चिकित्सा या बीमा पहचान पत्र;

(पांच) भारतीय पासपोर्ट;

(छह ) किसी मान्यताप्राप्त स्कूल शिक्षा बोर्ड द्वारा जारी मैट्रिकुलेशन यां 10वीं कक्षा या उच्चतर माध्यमिक या 12वीं कक्षा का प्रमाण-पत्र या अंकसूची;

(सात) किसी शासकीय सत्ता या सार्वजनिक क्षेत्र के उद्यम द्वारा सेवारत् या सेवानिवृत्त लोक सेवक को जारी किया गया पहचान पत्र या अन्य पहचान दस्तावेज;

(आठ) दिव्यांगजन अधिकार नियम, 2017 के अधीन अधिसूचित चिकित्सा प्राधिकरण द्वारा जारी दिव्यांगता प्रमाण-पत्र या दिव्यांगजन सशक्तिकरण विभाग , भारत सरकार द्वारा जारी विशिष्ट विकलांगता पहचान ( यूडीआईडी) कार्ड;

(नौ) सड़क परिवहन एवं राजमार्ग मंत्रालय भारत सरकार द्वारा जारी वाहन चालन अनुज्ञप्ति;

(दस) देखरेख और संरक्षण की आवश्यकता वाले बच्चे या विधिक मामले में संलिप्त बच्चे के संबंध में , जिसे किशोर न्याय (बालकों की देखरेख और संरक्षण ) अधिनियम , 2015 (2016 का 2) के अधीन उसके द्वारा बनाए गए नियमों के अधीन संबंधित राज्य शासन के साथ पंजीकृत चाइल्ड केयर इंस्टीट्यूशन में रखा जाता है , ऐसे संस्थान के प्रभारी व्यक्ति, अधीक्षक, बाल कल्याण अधिकारी या परिवीक्षा अधिकारी द्वारा जारी उसकी जन्म तिथि विनिर्दिष्ट करने वाला प्रमाण- पत्र; या

(ग्यारह) विदेशी नागरिक के संबंध में, -

(क) यदि वह ओवरसीज सिटीजन ऑफ इंडिया कार्डधारक है, तो ओवरसीज सिटीजन ऑफ इंडिया कार्ड;

(ख) यदि वह तिब्बती शरणार्थी है , तो विदेशी क्षेत्रीय पंजीकरण कार्यालय (फारेनर्स रोजनल रजिस्ट्रेशन आफिस ) द्वारा जारी पंजीकरण प्रमाण-पत्र;

( ग ) यदि वहः नेपाल या भूटान का नागरिक है, तो नेपाल या भूटान का पासपोर्ट;

(घ) यदि वह ओवरसीज सिटीजन ऑफ इंडिया कार्डधारक, तिब्बती शरणार्थी या नेपाल या भूटान का नागरिक नहीं है, तो विदेशी पासपोर्ट के साथ भारतीय वीजा या वर्तमान में विधिमान्य या विधिमान्यता समाप्त हो चुके विदेशी पासपोर्ट के साथ भारत का दीर्घकालिक वीजा; या

( बारह) ऐसे लाभार्थी के संबंध में , जिसके पास विधिक संरक्षक है, विधिक संरक्षकता के साक्ष्य हेतु दत्तक ग्रहण आदेश या अन्य दस्तावेज , जो संरक्षक एवं प्रतिपाल्य अधिनियम , 1890 ( 1990 का 8 ) किशोर न्याय (बालकों की देखरेख और संरक्षण ) अधिनियम , 2015 ( 2016 का 2 ) राष्ट्रीय स्वपरायणता प्रमस्तिष्कघात , मानसिक मंदता और बहुनिशक्तताग्रस्त व्यक्ति कल्याण न्यास अधिनियम, 1999 ( 1999 का 4) या दिव्यांगजन अधिकार अधिनियम, 2016 ( 2016 का 49 ) और उक्त अधिनियमों के अधीन बनाए गए लागू नियमों एवं विनियमों के अधीन विधि के न्यायालय या सक्षम प्राधिकारी द्वारा जारी किया गया हो; या

(तेरह) कोई अन्य दस्तावेज, जो कि राज्य शारान के अधीन उक्त विभाग निर्दिष्ट करे ।

(4 ) राज्य शासन के अधीन उक्त विभागों द्वारा इस संबंध में पदाभिहित एक अधिकारी बिन्दु क्रमांक 1 के खण्ड ( 3 ) के अधीन प्रस्तुत दस्तावेजों या उनकी अन्तर्वस्तुओं को प्रमाणित करने वाली सूचना के संबंध में जांच करेगा , -

(क) माय आधार पोर्टल ( https://myaadhaar.uidai.gov.in/portal ) पर ईआईडी प्रस्तुत करके नामांकन अनुरोध की स्थिति यह पुष्टि करने के लिए कि ईआईडी विधिमान्य है और नामांकन अनुरोध अस्वीकार नहीं किया है ; और

(ख) अन्य दस्तावेज और इस प्रयोजन के लिए , किसी शासकीय सत्ता या ऐसे प्राधिकरण की सहायता ले सकता है और प्रस्तुत सूचना को उसके साथ साझा कर सकता है जो ऐसे दस्तावेजों में निहित सूचना की तैयारी या रखरखाव से संबंधित है।

2. लाभार्थियों को उक्त लाभों की सुविधाजनक ढंग से प्राप्त करने में सक्षम बनाने के लिए, मंत्रालय उक्त योजना के अधीन आधार नंबर की आवश्यकता के बारे में लाभार्थियों को जागरूक करने के लिए मीडिया के माध्यम से व्यापक प्रचार सुनिश्चित करने के लिए सभी आवश्यक कदम उठाएगा।

3. जहां किसी भी लाभार्थी के आधार नंबर का बायोमेट्रिक आधारित अधिप्रमाणन रीति अर्थात् फेसियल इमेज , फिंगरप्रिंट या आईरिस स्केन आधारित अधिप्रमाणन के माध्यम से अधिप्रमाणन किसी भी कारण से विफल हो जाता है, जैसे कि बायोमेट्रिक जानकारी की खराब गुणवत्ता, तो निम्नलिखित उपचारात्मक उपाय अपनाए जाएंगे , अर्थात्ः-

(क) यदि अधिप्रमाणन का कोई विशेष बायोमेट्रिक-आधारित तरीका सफल नहीं होता है , तो बायोमेट्रिक- आधारित अधिप्रमाणन का कोई अन्य तरीका, या वन-टाइम पासवर्ड ( ओटीपी) आधारित अधिप्रमाणन जहां भी संभव और स्वीकार्य हो , प्रस्तावित किया जाएगा;

(ख) ऐसे मामलों में, जहां अधिप्रमाणन के बायोमेट्रिक-आधारित या ओटीपी-आधारित तरीके संभव नहीं हैं , उक्त योजना के अधीन लाभ यथा स्थिति आधार सिक्योर क्विक रिस्पोंस (क्यू आर) कोड या आधार पेपरलेस ऑफलाइन ई - केवाईसी दस्तावेज पर यूआईडीएआई के डिजिटल हस्ताक्षर प्रमाण-पत्र के ऑफ़लाइन सत्यापन करके आधार नंबर की वास्तविकता स्थापित करने के पश्चात्, निम्नलिखित में से किसी प्रक्रिया के आधार पर प्रदान किए जा सकेंगे; अर्थात्ः-

( एक) आधार सिक्यूर क्विक रिस्पोंस ( क्यू आर ) कोड जिसमें आधार कार्ड , आधार पत्र (अर्थात्, आधार नंबर धारक को उसके आधार नंबर के सृजन पर जारी किया गया पत्र) या ई -आधार (अर्थात्, यूआईडीएआई की वेबसाइट से डाउनलोड करने योग्य या इसके एम आधार ऐप का उपयोग करके सुलभ) आधार पत्र की पासवर्ड-संरक्षित इलेक्ट्रॉनिक प्रति शामिल है , आधार क्यूआर स्कैनर या एमआधार ऐप का उपयोग करके क्यूआर कोड को स्कैन करके ऑफ़लाइन सत्यापन के माध्यम से इसकी प्रामाणिकता स्थापित होने के पश्चात्;

(दो) आधार पेपरलेस ऑफलाइन ई-केवाईसी दस्तावेज ( यूआईडीएआई की वेबसाइट से डाउनलोड किए जाने योग्य या इसके एमआधार ऐप का उपयोग करके सुलभ यूआईडीएआई की वेबसाइट पर इस संबंध में दिए गए विवरण के अनुसार , इस प्रयोजन के लिए संबंधित मंत्रालय या विभाग या योजना कार्यान्वयन अभिकरण द्वारा विकसित एप्लिकेशन के माध्यम से दस्तावेज पर यूआईडीएआई के डिजिटल हस्ताक्षर प्रमाण- पत्र के ऑफलाइन सत्यापन के माध्यम से इसकी प्रामाणिकता स्थापित होने के पश्चात्।

4. ऊपर अंतर्विष्ट किसी भी बात के होते हुए भी, -

(क) उक्त योजना के अधीन लाभ दिए जाने से किसी बच्चे को मना नहीं किया जाएगा, -

(एक) अधिप्रमाणन करके या आधार नंबर रखने का सबूत प्रस्तुत करके अपनी पहचान स्थापित करने में विफल रहने की दशा में ; या

(दो) नामांकन के लिए आवेदन प्रस्तुत करने की दशा में , जहां उसे कोई आधार नंबर नहीं दिया गया है; और

(ख) उक्त योजना के अधीन लाभ, ऐसे बच्चे को उसकी पहचान सत्यापित करके और उसके माता- पिता या कानूनी अभिभावक के साथ उसके संबंध को स्थापित करके, बिन्दु क्रमांक 1 के खण्ड ( 3 ) के परंतुक में यथा विनिर्दिष्ट रीति में दिया जाएगा; और

(ग) जहां बिन्दु क्रमांक 4 के खण्ड (ख ) के अधीन लाभ दिया जाता है, वहां उसके संबंध में अभिलेख संधारित किया जाएगा , जिसकी राज्य शासन के अधीन विभागों द्वारा समय- समय पर समीक्षा और लेखापरीक्षा की जाएगी।

Whereas , the use of Aadhaar number to establish identity enables individuals to receive subsidies , benefits and services in a convenient and seamless manner , obviates the need for multiplicity of documents to establish identity , simplifies processes and promotes transparency and efficiency ;

And whereas , the various departments under the Government of Madhya Pradesh ( hereinafter referred to as the said Ministry or the said Department or the said Government , as the case may be ) are administering various departmental Schemes / Services using Samagra ID on Samagra Platform for the purposes of beneficiary registration and verification ( Schemes as attached- Annexure 1 ) ;

And whereas , subsidy , benefit or service for receipt of which proof of Aadhaar number is proposed to be made necessary is given to the Individual / Beneficiary under the said Scheme and the instructions and guidelines issued in respect thereof. The expenditure for the said Scheme is incurred from the Consolidated Fund of the State of Madhya Pradesh ;

Now , therefore , in exercise of the powers conferred. by section 7 of the Aadhaar ( Targeted Delivery of Financial and Other Subsidies , Benefits and Services ) Act, 2016 ( 18 of 2016 ) read with rule 5 of the Aadhaar Authentication for Good Governance ( Social Welfare , Innovation , Knowledge ) Rules , 2020 , the State Government , hereby , notifies the use of Aadhaar मध्यप्रदेश राजपत्र, दिनांक 15 अप्रैल 2025

number for the purpose of establishing identity of a beneficiary as a condition for the receipt of the said benefit and requires that such beneficiary undergo authentication or furnish proof of possession of Aadhaar number or in case of an individual , who has no Aadhaar number assigned to him shall submit an application for enrolment , as per the guidelines issued in this behalf , namely : -

1. (1) An individual desirous of availing the said benefit under the said Scheme shall be required to undergo authentication or furnish proof of possession of Aadhaar number .

(2) In case an individual has not been assigned an Aadhaar number , he shall be required to submit an application for enrolment :

Provided that if the individual is a child , such application shall be submitted only with the consent of his parent or legal guardian .

(3) In accordance with the provisions of regulation 12 of the Aadhaar ( Enrolment and Update ) Regulations , 2016 , the said Departments under the Government of Madhya Pradesh shall ensure enrolment of such beneficiaries , who are yet to be enrolled or update their Aadhaar details through appropriate measures , including co - ordination with Registrars and setting up enrolment centres at convenient locations or providing enrolment facilities by becoming a Registrar itself :

Provided that till such time an Aadhaar number is assigned to such beneficiary , he may establish his identity to avail the said benefit by presenting the following documents to which he is entitled and which are valid at the time of presentation or in case the software provided or authorised by the said Departments under Government of Madhya Pradesh for such identification supports obtaining electronic information for the purpose of evidencing the contents of such documents from the database of the authorities dealing with the preparation or maintenance thereof by giving his consent for so obtaining , namely :-

For a child below 18 years of age to whom an Aadhaar number has not been assigned:

(a) the acknowledgement of the beneficiary who has undergone the process of enrolment , provided by the operator at the enrolment centre , containing the Enrolment ID ( EID) ;

(b) any one of the following documents to evidence that the beneficiary is a child below 18 years of age, namely:

(i) certificate of birth provided under the Registration of Births and Deaths Act, 1969 (18 of 1969 ) , as extracted from the entry made in the register of births regarding the birth of the beneficiary ;

(ii) Indian passport ;

(iii) statement certificate or of marks of Matriculation or 10th Class or Higher Secondary or 12th Class , issued by any recognised Board of School Education ;

(iv) in respect of a child in need of care and protection or a child in conflict with law , who is housed by Child Care Institution registered with the concerned State Government under the rules made by it under the Juvenile Justice ( Care and Protection of Children) Act , 2015 (2 of 2016 ) , certificate specifying his date of birth issued by the Person - In - Charge , Superintendent , Child Welfare Officer or Probation Officer of such Institution ; or

(v) in respect of a foreign national ,-

(A) if he is an Overseas Citizen of India Cardholder , then Overseas Citizen of India Card;

(B) if he is a Tibetan refugee , then registration certificate issued by a Foreigners Regional Registration Office ;

passport or a Long Term Visa to India along with currently valid or expired foreign passport ; or

(c) any one of the following documents , having the photograph of the beneficiary, to evidence his relationship with the parent or legal guardian , namely: -

(i) Ration card;

(ii) Caste certificate or domicile certificate , issued by a Gazetted officer who is an Executive Magistrate or a Revenue Officer of the State Government , not below the rank of Tehsildar;

( iii ) medical or insurance identity card issued by a Government entity or public sector enterprise to a retired or serving public servant or his family member ;

(iv) Indian passport ;

(v) certificate or statement of marks of - Matriculation or 10th Class or Higher Secondary or 12th Class , issued by any recognised Board of School Education ;

(vi) in respect of a child in need of care and protection or a child in conflict with law , who is housed by Child Care Institution registered with the concerned State Government under the rules made by it under the Juvenile Justice ( Care and मध्यप्रदेश राजपत्र, दिनांक 15 अप्रैल 2025

Protection of Children ) Act , 2015 ( 2 of 2016 ) , certificate specifying his date of birth issued by the Person - In - Charge , Superintendent , Child Welfare Officer or Probation Officer of such Institution ; or

(vii) in respect of a foreign national ,

(A) if he is an Overseas Citizen of India Cardholder , then Overseas Citizen of India Card;

a (B) if he is Tibetan refugee , then registration certificate issued by a Foreigners Regional Registration Office ;

(D) ifhe is other than a Overseas Citizen of India Cardholder , Tibetan refugee or a national of Nepal or Bhutan , then either an Indian visa along with foreign passport or a Long Term Visa to India along with currently valid or expired foreign passport ; or

(2 of 2016) , the National Trust for Welfare of Persons with Autism , Cerebral Palsy , Mental Retardation and Multiple Disabilities Act , 1999 (44 of 1999 ) or the Rights of Persons with Disabilities Act , 2016 (49 of 2016 ) and the applicable rules and regulations made under the said Acts ; or

(ix) any other document , as the said Departments under the State Government , may specify.

For beneficiaries aged 18 years or more to whom an Aadhaar number has not been assigned:

(a) the acknowledgement of the beneficiary having undergone the process of enrolment , provided by the operator at the enrolment centre , containing the EID ;

(b) any one of the following documents , having the beneficiary's photograph , namely: -

(1) Elector's Photo Identity Card issued by the Election Commission of India;

(ii) Ration card ;

(iii) caste certificate or domicile certificate , issued by a Gazetted Officer , who is an Executive Magistrate or a Revenue Officer of the State Government , not below the rank of Tehsildar;

(iv) medical or insurance identity card issued by a Government entity or public sector enterprise to a retired or serving public servant or his family member ;

(viii ) disability certificate issued by notified medical authority under Rights of Persons with Disabilities Rules , 2017 , or Unique Disability Identification ( UDID) cardissued by the Department of Empowerment of Persons with Disabilities ( Divyangjan) , Government of India ;

( ix) driving licence issued by the Ministry of Road Transport and Highways , Government ofIndia:

(x) in respect of a child in need of care and protection or a child in conflict with law , who is housed by Child Care Institution registered with the concerned State Government under the rules made by it under the Juvenile Justice ( Care and Protection of Children ) Act , 2015 (2 of 2016 ) , certificate specifying his date of birth issued by the Person - In - Charge , Superintendent , Child Welfare Officer or Probation Officer of such Institution ; or मध्यप्रदेश राजपत्र, दिनांक 15 अप्रैल 2025

(xi) in respect of a foreign national ,-

(xiii) any other document, as the said Departments under the State Government , may specify .

(4) An officer designated by the said Departments under the State Government in this behalf shall check in respect of the documents presented or the information evidencing the contents thereof under clause (3) of point number 1 ,

(a) the status of the enrolment request by submitting the EID on myAadhaar portal (https : // myaadhaar.uidai.gov.in/portal) to confirm that the EID is valid and that the enrolment request does not stand rejected ; and

(b) the other documents and for this purpose , may take the assistance and share the information presented with any Government entity or an authority that deals with the preparation or maintenance of the information contained in such documents .

2. In order to enable beneficiaries to avail the said benefits conveniently , the Ministry shall make all necessary steps to ensure wide publicity through media to make the beneficiaries aware of the requirement of Aadhaar number under the said Scheme .

3. Where the authentication of the Aadhaar number of a beneficiary is done through any of the biometric- based modes of authentication , namely , facial image , fingerprints or iris scan based authentication , if it fails due to any reason , such as poor quality of biometric information , the following remedial mechanisms shall be adopted , namely:

(a) in case any particular biometric - based mode of authentication is not successful , any other mode of biometric - based authentication or One - Time Password (OTP) based authentication shall , wherever feasible and admissible , be offered ;

(b) in cases where biometric - based or OTP - based modes of authentication are not possible , benefits under the said Scheme may , after establishing the genuineness of the Aadhaar number by doing offline verification of the digital signature certificate of UIDAI on the Aadhaar Secure Quick Response ( QR) Code or the Aadhaar Paperless Offline e - KYC document , as the case may be , be given on the basis of any of the following , namely : - (1) an Aadhaar Secure Quick Response ( QR) Code containing Aadhaar card , Aadhaar letter ( i.e. , the letter issued to an Aadhaar number holder on generation of his Aadhaar number ) or e - Aadhaar ( i.e. , the password - protected electronic copy of Aadhaar letter downloadable from the website of UIDAI or accessible using its mAadhaar app ) , after its genuineness is established through offline verification by scanning the QR code using the Aadhaar QR Scanner or mAadhaar apps ;

(ii) Aadhaar Paperless Offline e - KYC document (downloadable from the website of UIDAI or accessible using its mAadhaar app ) , after its genuineness is established through offline verification of the digital signature certificate of UIDAI on the document through the application developed by the Ministry or Department or Scheme implementing agency concerned for this purpose , in accordance with the details given in this regard on the website of UIDAI .

4. Notwithstanding anything contained hereinabove ,-

(a) benefit under the said Scheme shall not be denied to a child,-

(i) in case of failure to establish his identity by undergoing authentication or to furnish proof of possession of Aadhaar number ; or

(ii) in case of production of an application for enrolment where he has not been assigned an Aadhaar number ; and

(b) benefit under the said Scheme shall be given to such a child by verifying his identity and establishing his relationship with his parent or legal guardian in the manner as specified in the proviso to clause ( 3 ) of point number 1 ; and

(c) where benefit is provided under clause ( b) of point number 4 , a record shall be maintained in respect of the same , which shall be reviewed and audited periodically by the Departments under the State Government .

5. In order to ensure that bona fide beneficiaries who are aged 18 years or more are not deprived of the benefit due to them under the said Scheme , the Departments under the State Government shall follow the exception handling mechanism specified in the Office Memorandum no . D - 26011 / 04 / 2017- DBT , dated the 19th December , 2017 of the Direct Benefit Transfer Mission , Cabinet Secretariat , Government of India (available on https://dbtbharat.gov.in ) .

6 . This notification shall come into force from the date of its publication in the Madhya Pradesh Gazette .

यतः , इलेक्ट्रॉनिक एवं सूचना प्रौद्योगिक मंत्रालय, भारत सरकार ने अपने कार्यालय ज्ञापन क्र. 13 ( 2 ) /2021 - ईजी- दो, दिनांक 24 अगस्त, 2022 के माध्यम से पहचान के प्रयोजन के लिए स्वैच्छिक आधार पर आधार अधिप्रमाणन उपयोग के लिए समग्र पोर्टल में निवासियों की पहचान के लिए मध्यप्रदेश शासन , विज्ञान और प्रौद्योगिकी विभाग (एमपीएसईडीसी) को अनुमत करने के लिए सक्षम प्राधिकारी को अनुमोदन की जानकारी दी है;

और यतः , आधार (वित्तीय एवं अन्य सहायिकियों , लाभ और सेवाओं का लक्ष्यित परिदान) अधिनियम , 2016 ( 2016 का 18 ) की धारा 4 की उप - धारा (4 ) के खण्ड (ख) का उप- खण्ड (दो) स्वैच्छिक आधार पर अधिप्रमाणनं करने की अनुमति देता है , इस प्रयोजन के लिए जैसा कि प्राधिकरण के परामर्श से एवं हित में विहित करे और केन्द्र सरकार इस प्रयोजन के लिए भारत सरकार ने सुशासन के लिए आधार अधिप्रमाणन ( सामाजिक कल्याण, नवाचार, ज्ञान) नियमावली, 2020 विरचित किए हैं, जिसमें राज्य सरकार स्वैच्छिक आधार परं आधार उपयोग करने के लिए उक्त नियमों के अधीन अनुमति की वांछा कर सकती है। मध्यप्रदेश शासन , विज्ञान और प्रौद्योगिकी विभाग , आधार अधिप्रमाणन के उपयोग के संबंध में केन्द्र सरकार द्वारा यथाविहित दिशानिर्देशों का पालन करेगा तथा दावेदार के आधार का उपयोग करने का आशय न रखने की दशा में मध्यप्रदेश शासन, विज्ञान और प्रौद्योगिकी विभाग , पहचान के तरीके को पृथक रुप से अधिसूचित करेगा।

अतएव , सुशासन के लिए आधार (वित्तीय और अन्य सहायिकियों, प्रसुविधाओं और सेवाओं का लक्ष्यित परिदान) अधिनियमः 2016 ( 2016 का 18 ) की धारा 4 की उप - धारा (4) के खण्ड (ख) के उप- खण्ड (दो) के साथ पठित आधार अधिप्रमाणन ( सामाजिक कल्याण , नवाचार, ज्ञान) नियमावली, 2020 के नियम 5 द्वारा प्रदत्त शक्तियों को प्रयोग में लाते हुए, राज्य सरकार, एतद्द्वारा, लाभार्थी के पंजीकरण एवं सत्यापन के प्रयोजन के लिए स्वैच्छिक आधार पर समग्र पोर्टल पर समग्र आई डी का उपयोग करके आधार अधिप्रमाणन करने के लिए यहां इसके साथ परिशिष्ट -1 के रूप में संलग्न विभिन्न सरकारी सेवाओं तथा योजनाओं को अधिसूचित करती है ।

Whereas , the Government of India , Ministry of Electronics and Information Technology vide its Office Memorandum No. 13 ( 2 ) / 2021 - EG-II , dated 24th August , 2022 has conveyed the approval of the competent authority to allow Government of Madhya Pradesh , Science and Technology Department ( MPSEDC) for identification of residents in Samagra portal for using Aadhaar authentication on voluntary basis for the purpose of identification ;

And whereas , sub - clause ( ii ) of clause ( b) of sub - section (4) of section 4 of the Aadhaar (Targeted Delivery of Financial and Other Subsidies , Benefits and Services ) Act, 2016 ( 18 of 2016 ) , allows performing authentication on voluntary basis , for such purpose as the Central Government in consultation with the authority and in the interest , may prescribe and for such 232(30)

purposes the Government of India has framed the Aadhaar Authentication for Good Governance ( Social Welfare , Innovation , Knowledge) Rules , 2020 , wherein the State Government may seek permission under the said rules to use Aadhaar on voluntary basis . The Government of Madhya Pradesh , Science and Technology Department shall adhere to the guidelines with respect to the use of Aadhaar authentication as prescribed by the Central Government and in case the claimant does not intend to authenticate using Aadhaar , then the Government of Madhya Pradesh , Science and Technology Department shall separately notify the methods of identification ;

Now , therefore , in exercise of the powers conferred by rule 5 of the Aadhaar Authentication for Good Governance ( Social Welfare , Innovation , Knowledge ) Rules , 2020 read with sub- clause ( ii ) of clause ( b ) of sub - section (4) of section 4 of the Aadhaar ( Targeted Delivery of Financial and Other Subsidies , Benefits and Services ) Act, 2016 (18 of 2016) , the State Government , hereby , notifies that for various Government Services and Schemes enclosed herewith as Annexure - 1 , to perform Aadhaar authentication on voluntary basis using Samagra ID on Samagra portal for the purposes of beneficiary registration and verification .
