No. SPP.3219/442/C.R. 63/Desk-14.— In exercise of the powers conferred by section 24 (8) Criminal Procedure Code, section 8 of Maharashtra Control of Organised Crime Act, 1999 the Government of Maharashtra is hereby pleased to designate Adv. Shri Bhushan R. Salvi, Assistant Government Pleader and Additional Public Prosecutor, as Special Public Prosecutor to conduct the cases under M.C.O.C. Act before Sessions Court, Raigad with effect from date of notification until further orders.

2. His appointment as Special Public Prosecutor will be Co-terminus with his appointment as Assistant Government Pleader and Additional Public Prosecutor.

3. His appointment is strictly subject to the conditions of service laid down in the Maharashtra Law Officers (Appointment, Conditions of Service and Remuneration Rules, 1984).

4. The Government reserves the right to revoke/modify/annul this order without assigning any reasons.

Hemlata A. Deshmukh as '' Special Public Prosecutor '' for conducting the Sessions Case No. 338/2018 pending before Additional Sessions Court, Kalyan, Dist. Thane, which is arising out of C.R.No. I-276/2018 registered at Kolsewadi Police Station, Dist. Thane.

2. Her appointment is strictly subject to the conditions of service laid down in the Maharashtra Law Officers (Appointment, Conditions of Service and Remuneration) Rules, 1984.

3. Lump sum fees of Rs. 30,000 (Rupees Thirty Thousand Only) as quoted by Adv. Hemlata A. Deshmukh has been already deposited by the applicant Shri Kishore Barku Gaikwad to the Government, vide R.B.I. Challan No. 4305, dated 13th March 2019. The amount will be paid to Adv. Hemlata A. Deshmukh after completion of the case.

क्रमांक पी-१(१)/(ऑक्टोबर, २०१५)/एक.---खाली नमूद करण्यात आलेली विभागीय परीक्षा त्याच्या नावासमोर दर्शविलेल्या कालावधीत घेण्यात येईल :---

उपरोक्त विभागीय परीक्षेकरिता आयोगाच्या दिनांक १६ फेब्रुवारी २०१९ रोजीच्या पत्राच्या अनुषंगाने संबंधित विभाग प्रमुख कार्यालयाकडून प्राप्त झालेल्या उमेदवारांच्या नावांच्या यादीमधील उमेदवारांना प्रस्तुत परीक्षेसाठी प्रवेश देण्यात येणार आहे. यास्तव, प्रस्तुत परीक्षेचे वेळापत्रक विभाग प्रमुखांना पाठविण्यात येईल.

टीपा.---- (१) ज्या उमेदवारांना आयोगाने परीक्षेस बसण्यास परवानगी दिली आहे, त्यांना त्यांच्या वरिष्ठांमार्फत परीक्षा प्रवेशपत्र देण्यात येईल. मूळ प्रवेशपत्र तसेच मूळ ओळखपत्र दाखविल्याशिवाय उमेदवारांना परीक्षा कक्षात व परीक्षेस प्रवेश मिळणार नाही. तसेच परीक्षेच्या वेळी उमेदवारांनी स्वत:ची ओळख (Identification) पटवून देण्याकरिता स्वत:चे पॅनकार्ड, आधारकार्ड, निवडणूक आयोगाचे ओळखपत्र, पासपोर्ट किंवा ड्रायव्िंहग लायसेन्स (फक्त स्मार्ट कार्ड प्रकारचे) यांपैकी ' किमान एक ' ओळखपत्र मूळ प्रतीत व त्याची एक छायांकित प्रत तसेच कार्यालयाने दिलेले स्वत:चे ओळखपत्र मूळ प्रतीत सोबत आणणे आवश्यक आहे.

(२) विभागीय परीक्षांचे निकाल घोषित झाल्यानंतर ज्या उमेदवारांना गुणांची पडताळणी करावयाची आहे, अशा उमेदवारांनी निकालाच्या दिनांकापासून एक महिन्याच्या आत विभाग प्रमुखांकडे अर्ज पाठविणे आवश्यक आहे. उमेदवारांकडून प्राप्त झालेले अर्ज विभाग प्रमुखांनी आयोगाकडे पाठविताना त्यांनी विहित मुदतीत अर्ज केला आहे व गुणांच्या पडताळणी शुल्कापोटी प्रत्येक विषयास रुपये १०० प्रमाणे कोर्ट फी स्टॅम्प लावले आहेत, याची विभाग प्रमुखांनी प्रथम खात्री करूनच ते उमेदवारांच्या अर्जाच्या तारखेपासून १५ दिवसांच्या आत आयोगाकडे पाठविणे आवश्यक आहे. उमेदवारांकडून विहित मुदतीत विहित शुल्कासह विभाग प्रमुखांमार्फत न आलेल्या अर्जाचा आयोगाकडून विचार केला जाणार नाही. याची सर्व उमेदवारांनी नोंद घ्यावी.

(३) विभागीय परीक्षेच्या पारंपरिक स्वरूपाच्या उत्तरपुस्तिकेकरिता गुणांची पडताळणी करण्यात येते. फेरमुल्यांकन केले जात नाही.

सूचना.-----(१) एखाद्या विषयात/विषयसमुहात मागील परीक्षेस जर सूट मिळाली असेल व त्यामुळे विभाग प्रमुखांनी अशा उमेदवारास संबंधित विषयासाठी/विषयसमुहासाठी प्रवेश दिला नसतानाही तो त्या विषयास/विषयसमुहास पुन्हा बसला तर पूर्वी मिळालेली सूट रद्द ठरविण्यात येईल.

(२) ज्या उमेदवारांना आयोगाने परीक्षेस बसण्यास परवानगी दिली आहे, त्यांना त्यांच्या वरिष्ठांमार्फत परीक्षा प्रवेशपत्र व उमेदवारांना सूचनांची प्रत देण्यात येईल. मूळ प्रवेशपत्र तसेच मूळ ओळखपत्र दाखविल्याशिवाय उमेदवारांना परीक्षा कक्षात व परीक्षेस प्रवेश मिळणार नाही. तसेच परीक्षेच्या वेळी उमेदवारांनी स्वत:ची ओळख (Identification) पटवून देण्याकरिता स्वत:चे पॅनकार्ड, आधारकार्ड, निवडणूक आयोगाचेे ओळखपत्र, पासपोर्ट किंवा ड्रायव्िंहग लायसेन्स (फक्त स्मार्ट कार्ड प्रकारचे) यांपैकी '' किमान एक '' ओळखपत्र मूळ प्रतीत व त्याची एक छायांकित प्रत साक्षांकित केलेली तसेच कार्यालयाने दिलेले स्वत:चे ओळखपत्र मूळ प्रतीत सोबत आणणे आवश्यक आहे. (३) उत्तरे लिहिण्यासाठी मुख्य उत्तरपुस्तिकेव्यतिरिक्त पुरवणी म्हणून अन्य उत्तरपुस्तिका उपलब्ध करून दिली जाणार नाही. (४) ज्या प्रश्नपत्रिकांची उत्तरे पुस्तकांच्या सहाय्याने लिहावयाची आहेत. अशा वेळी उमेदवारांनी विभागाने प्रमाणित केलेली शासन प्रकाशित अधिकृत पुस्तके (स्वत:ची) (यथास्थिती केवळ बेअरअॅक्ट्स आणि/किंवा रुल्स) वापरावी. परंतु परीक्षेसाठी असलेल्या कोणत्याही प्रश्नपत्रिकेवरील टिपण्या असलेली पुस्तके, मार्गर्दशिका किंवा पेजर, मोबाईल फोन, प्रोग्रामेबल कॅलक्युलेटर वा नकाशे, इत्यादी वस्तू परीक्षा कक्षामध्ये आणण्याची उमेदवारांना सक्त मनाई आहे. सदर सूचनेचे उल्लंघन झाल्याचे आढळल्यास ती बाब शिस्तभंग असल्याचे समजण्यात येईल व आयोगामार्फत तसेच विभागामार्फत कारवाई केली जाईल. उमेदवारांना आयोगाकडून कोणतीही पुस्तके पुरविली जाणार नाहीत.

(५) परीक्षेच्या नियमामध्ये प्रमाणित केल्याप्रमाणे उमेदवारांना प्रश्नांची उत्तरे इंग्रजी किंवा मराठीमध्ये लिहिण्याची मुभा आहे. परंतु उत्तरपुस्तिकेत ज्या भाषेत उत्तर लिहिणे सुरू केले असेल, त्याच भाषेत त्या उत्तरपुस्तिकेत शेवटपयर्ंत उत्तरे लिहिली पाहिजेत. परीक्षेच्या प्रश्नपत्रिका मराठी व इंग्रजी भाषेतच काढल्या जातील.

(६) उमेदवाराने प्रश्नांचे उप प्रश्न सोडविताना (अ, ब, क, ड किंवा १, २, ३, ४) सलगरित्या सोडवावेत. प्रश्नाचा एक भाग एका पृष्ठावर तर दुसरा भाग काही पृष्ठे सोडून अथवा काही अन्य पृष्ठावर उप प्रश्न सोडविल्यास तो उप प्रश्न दुर्लक्षित करण्यात येईल व अशा प्रश्नांच्या उत्तरांना शून्य गुण समजण्यात येतील.

(७) (अ) सदर परीक्षेच्या प्रश्नपत्रिकेतील प्रश्नांमध्ये काही दोष आढळल्यास, त्यासंबंधीचे लेखी निवेदन उमेदवाराने उप सचिव, (परीक्षा २) महाराष्ट्र लोकसेवा आयोग, मुंबई ४०० ०२१ यांच्याकडे स्वत:च्या तपशिलासह टपालाने परीक्षेच्या दिनांकापासून आठ दिवसांच्या आत विभाग प्रमुखांमार्फत सादर करावे. तद्नंतर आलेली निवेदने विचारात घेतली जाणार नाहीत. या दोषांबाबत पर्यवेक्षक/समवेक्षक यांच्याकडे विचारणा किंवा चर्चा करू नये.

(ब) उमेदवारांनी विभागीय परीक्षेशी संबंधित प्रश्नपत्रिकेतील त्रुटी संदर्भातील पत्रव्यवहार वगळता अन्य पत्रव्यवहार थेट आयोगाशी न करता तो विभाग प्रमुखांमार्फत आयोगाचे सचिव, महाराष्ट्र लोकसेवा आयोग, मुंबई यांचे नावे करावा, याची संबंधित उमेदवारांनी नोंद घ्यावी.

(८) विभागीय परीक्षांचे निकाल घोषित झाल्यानंतर ज्या उमेदवारांस गुणांची पडताळणी करून घ्यावयाची आहे अशा उमेदवारांनी निकालाच्या दिनांकापासून एक महिन्याच्या आत विभागप्रमुखांकडे अर्ज पाठविणे आवश्यक आहे. उमेदवारांकडून प्राप्त झालेले अर्ज विभाग प्रमुखांनी आयोगाकडे पाठवितांना त्यांनी विहित मुदतीत अर्ज केला आहे व गुणांच्या पडताळणी शुल्कापोटी प्रत्येक विषयास रुपये १०० प्रमाणे कोर्ट फी स्टॅम्प लावले आहेत, याची विभाग प्रमुखांनी प्रथम खात्री करूनच ते उमेदवारांच्या अर्जाच्या तारखेपासून १५ दिवसांच्या आत आयोगाकडे पाठविणे आवश्यक आहे. उमेदवारांकडून विहित मुदतीत विहित शुल्कासह विभागप्रमुखांमार्फत न आलेल्या अर्जाचा आयोगाकडून विचार केला जाणार नाही याची सर्व उमेदवारांनी नोंद घ्यावी.

(९) विभागीय परीक्षेच्या पारंपरिक स्वरूपाच्या उत्तरपुस्तिकेकरिता गुणांची पडताळणी करण्यात येते. फेरमुल्यांकन केले जात नाही.

(१०) परीक्षेत अनधिकृत साहित्याआधारे नक्कल करण्यास किंवा दुसया उमेदवाराच्या उत्तरपत्रिकेवरून नक्कल करण्यास वा दुसया उमेदवारांकडून नियमबा मदत घेण्यास किंवा दुसया उमेदवाराला नियमबा मदत देण्यास अथवा तसा प्रयत्न करण्यास अथवा वारंवार आपल्या जागेवरून उठण्यास सक्त मनाई आहे. खाजगी प्रकाशनाची पुस्तके वापरल्यास या सूचनेचे उल्लंघन करणारा उमेदवार शिस्तभंग कारवाईच्या तरतुदीअन्वये कारवाईस पात्र ठरेल.

(११) आयोगाची उत्तरपुस्तिका ही आयोगाची मालमत्ता असून ती अनधिकृतरित्या हाताळणे वा स्वत:जवळ बाळगणारी व्यक्ती कारवाईस पात्र ठरेल.

(१२) आयोगाने विभागीय परीक्षा संदर्भाने केलेल्या सूचनांचे उमेदवाराने अत्यंत काळजीपूर्वक पालन करावे. सूचनांचे पालन न करणारा उमेदवार परीक्षा कक्षाबाहेर घालवून देणे, गुण वजा करणे, पुढील परीक्षांना प्रतिरोधित करणे, उमेदवारी रद्द करणे, इत्यादी शिक्षा देण्यास पात्र ठरेल. शिक्षा देण्याचा अधिकार सर्वस्वी आयोगाचा राहील.

(२) सदरचे अधिकार सहकार कायदा कलम १०१ व कलम ९१ अन्वये, प्राप्त वसुली दाखल्याची अंमलबजावणी करण्याकरिता असतील.

(३) वसुली अधिकारी यांनी या कार्यालयाने दिनांक २४ जून, २०१६ रोजी निर्गमित केलेल्या परिपत्रकानूसार दरमहा विहित नमुन्यात 'अ-३', 'अ-४' व 'ब' कर्ज वसुलीची माहिती जिल्हा उप निबंधक, सहकारी संस्था, सोलापूर यांना सादर करावी.

(४) वसुली अधिकार प्रदान केलेल्या व्यक्तीची बदली झाल्यास किंवा सदर व्यक्ती सेवा निवृत्ती झाल्यास किंवा सदर व्यक्तीस सेवेतून काढून टाकल्यास किंवा त्याचा मृत्यू झाल्यास, अशा अधिकायास प्रदान केलेले अधिकार आपोआप रद्द होतील.

(५) ''मा. उच्च न्यायालय'', मुंबई यांनी रिट याचिका क्रमांक १९४२/२०१३, दि बेसिन कॅथोलिक को-ऑप. बँक विरुद्ध महाराष्ट्र राज्य व इतर या याचिकेमध्ये, दिनांक ५ डिसेंबर २०१५ रोजी दिलेल्या आदेश विचारात घेऊन वसुलीची कार्यवाही करताना महाराष्ट्र सहकारी संस्था अधिनियम, १९६० व नियम, १९६१ अन्वये प्राप्त असलेल्या अधिकारानुसारच वसुलीची कार्यवाही करावी.

(६) वसुली अधिकारी यांनी नमूद केलेल्या कालावधीत वसुलीचे कामकाज समाधानकारक केले नसल्याचे निदर्शनास आल्यास किंवा प्रदान केलेल्या अधिकाराचा गैरवापर करीत असल्याचे निदर्शनास आल्यास, प्रदान केलेले अधिकार काढून घेतले जातील व भविष्यात असे कोणतेही अधिकार अशा व्यक्तीस प्रदान करण्यात येणार नाही.

(७) शासन निर्णय क्रमांक संकीर्ण १५०४/प्र.क्र.२३१/१५-स, दिनांक २३ नोव्हेंबर, २००६ अन्वये वसुली अधिकायांना तीन सिंहाच्या राजमुद्रेचा वापर करता येणार नाही.

(८) वसुली अधिकायास त्याच्या नावापुढे कोठेही ""महाराष्ट्र शासन नियुक्त"" आणि किंवा "" सिव्हील कोर्ट"", ""एक्झीक्युटीव्ह कोर्ट"" या शब्दांचा अथवा तत्सम शब्दांचा वापर करता येणार नाही. त्याऐवजी वसुली अधिकायंानी त्यांच्या नावापुढे "" वसुली अधिकारी (महाराष्ट्र सहकारी संस्था अधिनियम, १९६० व नियम, १९६१ चे नियम, १०७ अन्वये)"" असे लिहावे.

(९) वसुली अधिकायास वसुलीसाठी वापर करण्यात येणाया वाहनावर ""वसुली अधिकारी, विकास सहकारी बँक लि., सोलापूर"" असा फलक लावता येऊ शकेल.

(१०) वसुली अधिकायाने पगार जप्तीची कार्यवाही करताना कर्जदाराच्या/जामीनदाराच्या पगार कपातीबाबत राष्ट्रीयीकृत बँका वा अन्य बँका यांच्याकडे परस्पर वेतन कपातीचा आदेश देऊन परस्पर पगार कपात करता येणार नाही. संबंधित कर्जदार/जामीनदार ज्या आस्थापनेवर काम करीत आहे त्या आस्थापनेवरील आहरण व संवितरण अधिकारी/पगार करणारा अधिकारी यांचेमार्फत पगारातून कर्ज वसूल करण्याची कार्यवाही करावी.

(११) वसुली अधिकायास, थकबाकीदाराकडून वसुली दाखल्यानुसार वसूलपात्र रक्कम व या कार्यालयाचे कलम १५६ अन्वयेचे आदेश दिनांक ६ एप्रिल २०१८ मध्ये नमूद केलेप्रमाणे व दराने अंमलबजावणी खर्चाची रक्कम याशिवाय इतर कोणतीही रक्कम वसूल करता येणार नाही.

(१२) या कार्यालयाचे आदेश/परिपत्रक दिनांक १ जानेवारी, २०१७ मधील क्र. ४ कलम १५६ व नियम, १०७ अन्वये वसुली अधिकायांनी करावयाची कार्यवाही यामधील मार्गदर्शक सूचनांचे पालन करावे. बँकेचे मुख्य कार्यकारी अधिकारी यांनी दरमहा वसुली अधिकायांचे कामकाजाचा कर्जदारनिहाय आढावा परिपत्रकातील प्रपत्र-पाच मध्ये नमूद केल्याप्रमाणे घ्यावा.

अटी :---

(१) वसुली अधिकारी यांनी सदरचे अधिकार जनता सहकारी बँक लि. पुणे (मल्टीस्टेट शेडयुल्ड बँक) ही बँक बहुराज्यीय बँकेत रुपांतरीत होण्यापूर्वी उक्त बँकेने अदा केलेल्या कर्ज प्रकरणापुरतेच वापरता येतील. अन्यथा इतर प्रकारे सदर अधिकाराचा वापर करता येणार नाही.

(२) सदरचे अधिकार सहकार कायदा कलम १०१ व कलम ९१ अन्वये, प्राप्त वसुली दाखल्याची अंमलबजावणी करण्याकरिता असतील.

(३) महाराष्ट्र सहकारी संस्था अधिनियम, १९६० चे कलम १५६ व नियम, १९६१ चे नियम १०७ मधील तरतुदीनुसार कर्ज वसुलीची कार्यवाही करावी. कर्ज वसुली करताना सहकार कायदा, नियम यातील तरतुदी व परिपत्रकीय सूचनांचे पालन न केल्याचे निदर्शनास आल्यास, आवश्यक ती चौकशी करून आवश्यकता भासल्यास, सदरचे प्रदान केलेले अधिकार तत्काळ काढून घेण्यात येतील.

(४) वसुली अधिकार यांनी मालमत्तेच्या वाजवी किंमतीस (अपसेट प्राईज) निबंधकाची पूर्व मान्यता घेणे आणि मालमत्तेची विक्री कायम करणे या कामी या कार्यालयाचे परिपत्रक जा.क्र. ना.बँका/डी-४/अपसेट प्राईज विक्री कायम/२०१५/८७०, दिनांक १७ एप्रिल २०१५ अनुसार संबंधित जिल्हा उप निबंधक, सहकारी संस्था यांची मान्यता घ्यावी संबंधित जिल्हा उप निबंधक सहकारी संस्था यांनी उक्त बँकेच्या वाजवी किंमतीस (अपसेट प्राईज) पूर्व मान्यता देणे आणि मालमत्तेची विक्री कायम करणे सबंधी प्रस्तावाबाबत महाराष्ट्र सहकारी संस्था नियम, १९६१ चे नियम १०७ मधील तरतुदीनुसार उचित कार्यवाही करावी.

(५) वसुली अधिकारी यांनी दर महा या कार्यालयाचे परिपत्रक दिनांक २४ जून, २०१६ अन्वये विहित केलेल्या नमुन्यात 'अ-३', 'अ-४' व 'ब' कर्ज वसुलीची माहिती संबंधीत जिल्हा उप निबंधक, सहकारी संस्था, यांच्या कार्यालयात सादर करावी.

(६) प्रदान केलेल्या व्यक्तीची बदली झाल्यास किंवा सदर व्यक्ती सेवा निवृत्ती झाल्यास किंवा सदर व्यक्तीस सेवेतून काढून टाकल्यास किंवा त्याचा मृत्यू झाल्यास, अशा अधिकायास प्रदान केलेले अधिकार आपोआप रद्द होतील.

(७) ''मा. उच्च न्यायालय'', मुंबई यांनी रिट याचिका क्रमांक १९४२/२०१३, दि बेसिन कॅथोलिक को-ऑप. बँक विरुद्ध महाराष्ट्र राज्य व इतर या याचिकेमध्ये, दिनांक ५ डिसेंबर २०१५ रोजी दिलेल्या आदेश विचारात घेऊन वसुलीची कार्यवाही करताना महाराष्ट्र सहकारी संस्था अधिनियम, १९६० व नियम, १९६१ अन्वये प्राप्त असलेल्या अधिकारानुसारच वसुलीची कार्यवाही करावी.

(८) वसुली अधिकारी यांनी नमूद केलेल्या कालावधीत वसुलीचे कामकाज समाधानकारक केले नसल्याचे निदर्शनास आल्यास किंवा प्रदान केलेल्या अधिकाराचा गैरवापर करीत असल्याचे निदर्शनास आल्यास, प्रदान केलेले अधिकार काढून घेतले जातील व भविष्यात असे कोणतेही अधिकार अशा व्यक्तीस प्रदान करण्यात येणार नाही.

(९) शासन निर्णय क्रमांक संकीर्ण १५०४/प्र.क्र.२३१/१५-स, दिनांक २३ नोव्हेंबर, २००६ अन्वये वसुली अधिकायांना तीन सिंहाच्या राजमुद्रेचा वापर करता येणार नाही.

(१०) वसुली अधिकायास त्याच्या नावापुढे कोठेही ""महाराष्ट्र शासन नियुक्त"" आणि किंवा "" सिव्हील कोर्ट"", ""एक्झीक्युटीव्ह कोर्ट"" या शब्दांचा अथवा तत्सम शब्दांचा वापर करता येणार नाही. त्याऐवजी वसुली अधिकायंानी त्यांच्या नावापुढे "" वसुली अधिकारी (महाराष्ट्र सहकारी संस्था अधिनियम, १९६० व नियम, १९६१ चे नियम, १०७ अन्वये)"" असे लिहावे.

(११) वसुली अधिकायास वसुलीसाठी वापर करण्यात येणाया वाहनावर ""वसुली अधिकारी, जनता सहकारी बँक लि., पुणे (मल्टीस्टेट शेडयुल्ड बँक)"" असा फलक लावता येऊ शकेल.

(१२) वसुली अधिकायाने पगार जप्तीची कार्यवाही करताना कर्जदाराच्या/जामीनदाराच्या पगार कपातीबाबत राष्ट्रीयकृत बँका वा अन्य बँका यांच्याकडे परस्पर वेतन कपातीचा आदेश देऊन परस्पर पगार कपात करता येणार नाही. संबंधित कर्जदार/जामीनदार ज्या आस्थापनेवर काम करीत आहे त्या आस्थापनेवरील आहरण व संवितरण अधिकारी/पगार करणारा अधिकारी यांचेमार्फत पगारातून कर्ज वसूल करण्याची कार्यवाही करावी.

(१३) वसुली अधिकायास, थकबाकीदाराकडून वसुली दाखल्यानुसार वसूलपात्र रक्कम व या कार्यालयाचे कलम १५६ अन्वयेचे आदेश दिनांक ६ एप्रिल २०१८ मध्ये नमुद केलेप्रमाणे व दराने अंमलबजावणी खर्चाची रक्कम याशिवाय इतर कोणतीही रक्कम वसूल करता येणार नाही.

(१४) या कार्यालयाचे आदेश/परिपत्रक दिनांक १ जानेवारी, २०१७ मधील क्र. ४ कलम १५६ व नियम, १०७ अन्वये वसुली अधिकायांनी करावयाची कार्यवाही यामधील मार्गदर्शक सूचनांचे पालन करावे. बँकेचे मुख्य कार्यकारी अधिकारी यांनी दरमहा वसुली अधिकायांचे कामकाजाचा कर्जदारनिहाय आढावा परिपत्रकातील प्रपत्र-पाच मध्ये नमूद केल्याप्रमाणे घ्यावा.
