By virtue of an affidavit sworn before the Executive Magistrate Bhubaneswar on Dated 19th March 2024, I, Mrs. Jyotshna Biswal, Address W/o-Ashok Kumar Biswal,Qrs. No. IVR-41, Unit- 6,Bhubaneswar,Dist. Khordha, Odisha have changed my name from "Jyotsna Mayee jena" to "Jyotshna Biswal". Henceforth, I shall be known as "Jyotshna Biswal" for all purposes.
