By virtue of an affidavit sworn before the Executive Magistrate Bhubaneshwar on Dated 12th March 2024, I, Mr. Sai Sumiran Das, Address VTC-Purusottampur, Post-Nalipur, District- Cuttack, Sate-Odisha, Pin-754205. have changed my name from "Simran Das" to "Sai Sumiran Das". Henceforth, I shall be known as "Sai Sumiran Das" for all purposes.
