By virtue of an affidavit sworn before the Notary Public Bhubaneswar on Dated 20th April 2024, I, Mrs. Phula Munda, Address Garamian, Kankadpal, Kaliapani, Jajpur, Odisha, 755028 have changed my name from "Phul Munda" to "Phula Munda". Henceforth, I shall be known as "Phula Munda" for all purposes.
