By virtue of an affidavit sworn before the Executive Magistrate Bhubaneswar on Dated 24th April 2024, I, Mr. Purusottam Nayak, Address Plot No- 1215/1486 B, Khandagiri Bari, Bhubaneswar have changed my name from "Purusottam Naik" to "Purusottam Nayak". Henceforth, I shall be known as "Purusottam Nayak" for all purposes.
