By virtue of an affidavit sworn before Executive Magistrate BHUBANESWAR on Dated 28th March 2024,I, Mr. SHAKTI PRASAD PATNAIK , Address C/O- Subash Ch Patnaik, Sibananda Nagar , Dura Bahadurpetta, Ganjam, Odisha-760007 have changed my Daughter's name from "TITIKSHA PATNAIK" to "TURVI PATNAIK". Henceforth, She shall be known as "TURVI PATNAIK" for all purposes.
