By virtue of an affidavit sworn before the Executive Magistrate Banki on Dated 22nd March 2024, I, Mrs. Usha Sahoo, Address At-Kuamada, Po-Banara, Ps-Banki, Dist-Cuttack have changed my name from "Oshi Sahoo" to "Usha Sahoo". Henceforth, I shall be known as "Usha Sahoo" for all purposes.
