By virtue of an affidavit sworn before Executive Magistrate Bhubaneswar on Dated 7th February 2024,I, Mr. Anoja Kumar Chhotaray , Address At/Po- Godiali , Baradandi, Ps- Bolagarh , Dist- Khordha , odisha have changed my Daughter's name from "SARBAPRIYA AYUSHI CHHOTARAY" to "SARBAPRIYA ARUSHI CHHOTARAY". Henceforth, She shall be known as "SARBAPRIYA ARUSHI CHHOTARAY" for all purposes.
