By virtue of an affidavit sworn before Notary Public Bhubaneswar on Dated 20th June 2024,I, Mrs. Sapuri Nayak , Address At-Gulipatna, P.O/P.S-Umercote, Dist.-Nabarangpur have changed my Daughter's name from "Sonali Nayak" to "Mamata Nayak". Henceforth, She shall be known as "Mamata Nayak" for all purposes.
