By virtue of an affidavit sworn before the Notary Public Bhubaneswar on Dated 18th June 2024, I, Mrs. Swarnalata Das, Address At-Paribasudeipur, PO-Bhatapatna ,PS-balianta, Khordha have changed my name from "Snehalata Das" to "Swarnalata Das". Henceforth, I shall be known as "Swarnalata Das" for all purposes.
