By virtue of an affidavit sworn before the Executive Magistrate Bhubaneswar on Dated 5th June 2024, I, Mrs. Rajalaxmi Patra, Address Plot No-1328, Bhimatangi, kapilaPrasad, , Ekamra College Road, PO: Old Town, PS:Air field, Bhubaneswar,751002 have changed my name from "Rajalaxmi Pradhan" to "Rajalaxmi Patra". Henceforth, I shall be known as "Rajalaxmi Patra" for all purposes.
