By virtue of an affidavit sworn before Bhubaneswar on Dated 1st July 2024,I, Mrs. Rojalini Rout, Address At- Plot No 353/4372, Nayapalli, Bhubaneswar, Jaydev Vihar, IRC Village, Dist- Khordha, Odisha, PIN- 751015, Odisha, have changed my Son's name from "Laxmikanta Tripathy" to "Rudra Narayan Patra". Henceforth, He shall be known as "Rudra Narayan Patra" for all purposes.
