By virtue of an affidavit sworn before the Notary Public Sambalpur on Dated 6th September 2024, I, Mrs. SHIRISHA AMUDALA, Address Qtr.no. C-61, Jagruti Vihar, MCL HQ, Burla, Sambalpur, Odisha, have changed my name from "DUVASI SHIRISHA" to "SHIRISHA AMUDALA". Henceforth, I shall be known as "SHIRISHA AMUDALA" for all purposes.
