By virtue of an affidavit sworn before the Executive Magistrate GOP on Dated 5th August 2024, I, Mr. KAILAS KANDI, Address AT-GIRIMA, PO-BIRATUNG, PS-GOP, DIST-PURI, Odisha, have changed my name from "KANDI KAILASH" to "KAILAS KANDI". Henceforth, I shall be known as "KAILAS KANDI" for all purposes.
