By virtue of an affidavit sworn before the Executive Magistrate Dhenkanal on Dated 15th February 2024, I, Mr. Jagat Rout, Address Village -Kashipur,P.O-Sarakapatana,Dist- Dhenkanal,Pin-759015, Odisha, Odisha, have changed my name from "Jugal Rout" to "Jagat Rout". Henceforth, I shall be known as "Jagat Rout" for all purposes.
