By virtue of an affidavit sworn before the Notary Public Bhubaneswar on Dated 4th September 2024, I, Mrs. Samapika Mohapatra, Address Flat No -202/ Block-A , Lifestylegreen Apartment , Near Kiit Square , Patia, Bhubaneswar, Odisha, have changed my name from "Mamuni Samapika Rout" to "Samapika Mohapatra". Henceforth, I shall be known as "Samapika Mohapatra" for all purposes.
