By virtue of an affidavit sworn before Executive Magistrate Kamakhyanagar on Dated 1st August 2024,I, Mr. Santosh Kumar Sahoo, Address Vill.-Kadua, P.O.-Bhagirathapur, P.S- Kamakhyanagar, Dist.-Dhenkanal, PIN-759018, Odisha, Odisha, have changed my Daughter's name from "Priyanshi Sahoo" to "Pratyasha Sahoo". Henceforth, She shall be known as "Pratyasha Sahoo" for all purposes.
