By virtue of an affidavit sworn before the Executive Magistrate BALASORE on Dated 6th September 2024, I, Mrs. SULOCHANA ROUT, Address AT-MATHASAHI, PO-MOTIGANJ, PS- TOWN, DIST-BALASORE-756003, ODISHA, Odisha, have changed my name from "SULOCHANA PRADHAN" to "SULOCHANA ROUT". Henceforth, I shall be known as "SULOCHANA ROUT" for all purposes.
