By virtue of an affidavit sworn before Notary Public Bhubaneswar on Dated 4th November 2024,I, Mr. Pratap Kumar Swain, Address Plot No 132/1226,Lane No 2,Millenium City, Pahal,Bhubaneswar, Odisha-752101, Odisha, have changed my Son's name from "Pradipto Swain" to "Parth Pratim Swain". Henceforth, He shall be known as "Parth Pratim Swain" for all purposes.
