LAVYA MURAKA, Address PLOT NO 1263/C SETOR 6 CDA CUTTACK, Odisha, have changed my name from "VAISHNAVI MURARKA" to "LAVYA MURAKA". Henceforth, I shall be known as "LAVYA MURAKA" for all purposes.
