By virtue of an affidavit sworn before the Executive Magistrate SALIPUR on Dated 24th July 2024, I, Mrs. KABITA DASH, Address VIL-CHANDRADEIPUR , PO/PS-Salipur, Dist-Cuttack, Odisha-754202, Odisha, have changed my name from "KABITA SARANGI" to "KABITA DASH". Henceforth, I shall be known as "KABITA DASH" for all purposes.
