By virtue of an affidavit sworn before the Executive Magistrate BHUBANESWAR on Dated 6th August 2024, I, Mr. SUDHAKAR MAHAPATRA, Address AT/PO-GOPALPUR,PS-RANPUR,DIST- NAYAGARH,ODISHA-752025, Odisha, have changed my name from "SUDARSHAN MOHAPATRA" to "SUDHAKAR MAHAPATRA". Henceforth, I shall be known as "SUDHAKAR MAHAPATRA" for all purposes.
