By virtue of an affidavit sworn before Notary Public Bhubaneswar on Dated 20th December 2024,I, Mr. Swapnil Parida, Address C/o Sushil Kumar Parida Plot No 1052/3345/1051/3346, Lane-4, Prasanti Vihar, PO-Baramunda colony Bhubaneswar Dist-Khordha 751003, Odisha, have changed my Daughter's name from "Shradha Suman" to "Shradha Suman Parida". Henceforth, She shall be known as "Shradha Suman Parida" for all purposes.
