By virtue of an affidavit sworn before Notary Public Bhubaneswar on Dated 20th December 2024,I, Mr. Swapnil Parida, Address C/o Sushil Kumar Parida Plot No 1052/3345/1051/3346, Lane-4, Prasanti Vihar, PO-Baramunda colony Bhubaneswar Dist-Khordha 751003, Odisha, have changed my Son's name from "Sai Swaroop" to "Sai Swaroop Parida". Henceforth, He shall be known as "Sai Swaroop Parida" for all purposes.
