By virtue of an affidavit sworn before the Executive Magistrate GOP on Dated 29th November 2024, I, Mr. PRATIK KUMAR SAHOO, Address VILLAGE/PO-KHADISA,PS-GOP,DIST-PURI,PIN- 752110, Odisha, have changed my name from "MINAKETAN SAHOO" to "PRATIK KUMAR SAHOO". Henceforth, I shall be known as "PRATIK KUMAR SAHOO" for all purposes.
