By virtue of an affidavit sworn before the Notary Public Cuttack on Dated 24th June 2024, I, Mrs. Supriya Sarangi, Address At.-Barasingha, Po.-Udranga, Ps.-Binjharpur, Dist.-Jajpur- 755014, Odisha, have changed my name from "Supriya Nayak" to "Supriya Sarangi". Henceforth, I shall be known as "Supriya Sarangi" for all purposes.
