By virtue of an affidavit sworn before the Notary Public Cuttack on Dated 25th November 2024, I, Mrs. Pratima Nayak, Address Plot No - 493, Talasahi New colony , Sikharpur Po- College square , Cuttack, Odisha, have changed my name from "Saraswati Nayak" to "Pratima Nayak". Henceforth, I shall be known as "Pratima Nayak" for all purposes.
