By virtue of an affidavit sworn before the Notary Public Bhubaneswar on Dated 4th November 2024, I, Mrs. Kirtti Chaurasia, Address Plot no.222/3, Jharana Lane, Bhimpur, Landmark: Behind Maa Shakti Hospital, Bhubaeswar-751020, Odisha, have changed my name from "Kirti Sharan Chaurasia" to "Kirtti Chaurasia". Henceforth, I shall be known as "Kirtti Chaurasia" for all purposes.
