By virtue of an affidavit sworn before Notary Public Pipil on Dated 24th January 2025,I, Mr. Mahanta Baliarsingh, Address At-Chainpur, Po-Motari, Ps-Delanga,Dist-Puri , pin-752015, Odisha, have changed my Son's name from "Surendra Baliyarsingh" to "Pintu Baliarsingh". Henceforth, He shall be known as "Pintu Baliarsingh" for all purposes.
