By virtue of an affidavit sworn before the Executive Magistrate Bhubaneswar on Dated 30th November 2024, I, Mr. Omsai Sanath Kumar Acharya, Address Permanent- At- Dasmankul, Po- Dhanua,PS-Tirtol, Dist-Jagatsinghpu, Pin-754137. Pr, Odisha, have changed my name from "Saisanat Acharya" to "Omsai Sanath Kumar Acharya". Henceforth, I shall be known as "Omsai Sanath Kumar Acharya" for all purposes.
