By virtue of an affidavit sworn before the Notary Public Bhubaneswar on Dated 4th November 2024, I, Mr. Abhischek Chaurasia, Address Plot No.222/3, Jharana Lane Bhimpur, Bhubaneswar-751020. Landmark: Behind Maa Shakti Hospital, Odisha, have changed my name from "Abhishek Chaurasia" to "Abhischek Chaurasia". Henceforth, I shall be known as "Abhischek Chaurasia" for all purposes.
