By virtue of an affidavit sworn before the Executive Magistrate Bhubaneswar on Dated 7th December 2024, I, Mrs. BIDYUTA PRAVA DEVI, Address House No. 158, Pratihari Sahi, At/Po/Ps- Sarankul, Dist-Nayagarh, Pin-752080, Odisha, have changed my name from "Bidyutalata Pratihari" to "BIDYUTA PRAVA DEVI". Henceforth, I shall be known as "BIDYUTA PRAVA DEVI" for all purposes.
