By virtue of an affidavit sworn before Executive Magistrate Bhubaneswar on Dated 9th January 2025,I, Mr. Rajesh Kumar Biswal, Address At/Po -Jagulaipada, Via- Jagulaipada,Ps- Rajkanika District: Kendrapara, Odisha - 754220, Odisha, have changed my Son's name from "Sidhartha Biswal" to "Om Sidhartha Biswal". Henceforth, He shall be known as "Om Sidhartha Biswal" for all purposes.
