By virtue of an affidavit sworn before the Executive Magistrate DARPAN on Dated 6th November 2024, I, Mrs. SABITA PARIDA, Address VILL-BARUNDAI, PO-BARUNDAI, PS-KORAI, DIST-JAJPUR, PIN-755025, STATE-ODISHA, Odisha, have changed my name from "SABITA DALAI" to "SABITA PARIDA". Henceforth, I shall be known as "SABITA PARIDA" for all purposes.
