By virtue of an affidavit sworn before the Notary Public Bhubaneswar on Dated 20th December 2024, I, Mr. Narendra Dadhicharan Mohanty, Address At.Plot No-113/3029, Udayagiri Vihar,near Mahalaxmi cold store patrapada Bhubaneswar dist-Khurda, Odisha, have changed my name from "Narendra D Mohanty" to "Narendra Dadhicharan Mohanty". Henceforth, I shall be known as "Narendra Dadhicharan Mohanty" for all purposes.
