By virtue of an affidavit sworn before the Executive Magistrate Sadar,Cuttack on Dated 13th December 2024, I, Miss. Jalilata sahu, Address At- Kainphulia sahi (Balramprasad) , Po- Balramprasad , Ps- NALCO , Block- Banarpal , Dist - Anugul , Pin Code - 759128, Odisha, have changed my name from "Shantilata sahu" to "Jalilata sahu". Henceforth, I shall be known as "Jalilata sahu" for all purposes.
