By virtue of an affidavit sworn before the Executive Magistrate SAMBALPUR on Dated 31st December 2024, I, Mrs. JAIMINI BHABASAGAR, Address AT/PO-GODLOISINGH ,PS-JUJOMURA ,DIST-SAMBALPUR ,PIN-768105, Odisha, have changed my name from "JAMINI BHABASAGAR" to "JAIMINI BHABASAGAR". Henceforth, I shall be known as "JAIMINI BHABASAGAR" for all purposes.
