By virtue of an affidavit sworn before the Executive Magistrate Puri on Dated 26th December 2024, I, Miss. Additi Mohanty, Address Gopala Ballava Road, Mati Mandap Sahi, Puri, Odisha, 752001, Odisha, have changed my name from "Aditi Mohanty" to "Additi Mohanty". Henceforth, I shall be known as "Additi Mohanty" for all purposes.
