By virtue of an affidavit sworn before the Executive Magistrate PATNAGARH on Dated 9th December 2024, I, Mr. DEBASHISH DASH, Address PHULJHAR, MALPADA, HARISHANKAR ROAD, BOLANGIR-767038, Odisha, have changed my name from "DEBASHISH DISHRI" to "DEBASHISH DASH". Henceforth, I shall be known as "DEBASHISH DASH" for all purposes.
