By virtue of an affidavit sworn before the Executive Magistrate Bhubaneswar on Dated 19th December 2024, I, Mrs. Sulochana Gochhayat, Address village/po-Itipur,ps- Dhauli,Bhubaneswar,Khordha,pin-751002, Odisha, have changed my name from "Tilotama Gochhayat" to "Sulochana Gochhayat". Henceforth, I shall be known as "Sulochana Gochhayat" for all purposes.
