By virtue of an affidavit sworn before the Executive Magistrate Patnagarh on Dated 9th December 2024, I, Mr. SANTOSH KUMAR DASH, Address PHULJHAR, MALPADA, HARISHANKAR ROAD, BOLANGIR-767038, Odisha, have changed my name from "SANTOSH KUMAR DISHRI" to "SANTOSH KUMAR DASH". Henceforth, I shall be known as "SANTOSH KUMAR DASH" for all purposes.
