By virtue of an affidavit sworn before the Notary Public NABARANGPUR on Dated 21st January 2025, I, Mr. PURUSTAM NAYAK, Address AT-DAHADAMAL PO-NUAKOTE PS-PAPADAHANDI DIST-NABARANGPUR, Odisha, have changed my name from "PURUSTAM HARIJAN" to "PURUSTAM NAYAK". Henceforth, I shall be known as "PURUSTAM NAYAK" for all purposes.
