By virtue of an affidavit sworn before the Executive Magistrate Bahanaga on Dated 27th January 2025, I, Mrs. Minati Nayak, Address W/o- Mahendra Prasad Mohanty, At/Po- Panpana, Via- Khantapada, Dist-Baleshwar, 756043, Odisha, have changed my name from "Minati Mohanty" to "Minati Nayak". Henceforth, I shall be known as "Minati Nayak" for all purposes.
