By virtue of an affidavit sworn before the Notary Public Bhubaneswar on Dated 22nd January 2025, I, Mrs. Santilata Swain, Address At-Pandua, P.o-Pandua, P.s-Tirtol, Dist-Jagatsinghapur, Odisha-754140, Odisha, have changed my name from "Santisudha Swain" to "Santilata Swain". Henceforth, I shall be known as "Santilata Swain" for all purposes.
