By virtue of an affidavit sworn before the Executive Magistrate Bhubaneswar on Dated 20th March 2025, I, Mr. Pratik Kumar Panda, Address Plot No.-262/2088, MAHADEV VIHAR, BHUBANESWAR, AIGINIA, PO: AIGINIA, DISTRICT- KHORDHA, ODISHA., Odisha, have changed my name from "P Pratik Kumar" to "Pratik Kumar Panda". Henceforth, I shall be known as "Pratik Kumar Panda" for all purposes.
