By virtue of an affidavit sworn before the Notary Public Bhubaneswar on Dated 11th February 2025, I, Mrs. Smrutishree Behera, Address D/o Anjan Behera, resident of At-Kandasar (Part), P.O-Kandasar, NALCO, Dist.-Anugul, PIN-759145, Odisha, Odisha, have changed my name from "Mandakini Behera" to "Smrutishree Behera". Henceforth, I shall be known as "Smrutishree Behera" for all purposes.
